If you have questions or concerns about your participation in the DS-Connect® registry, please contact us at: info@ds-connect.org. Please do not include any personal health information in your request.
- Click the button "Join the Registry" on the DS-Connect registry homepage or the "Join" menu in the upper right of the screen.
- On the Registration page, you will first "Create new account," which means entering your name and email address and creating a Username and Password.
- Tip: This Username and Password will be required each time you log into the account.
- Read the online consent questions.
- Check the appropriate consent/assent boxes.
- Complete the requested health details for the person with DS that will be the subject of the account.
- Click "Create new account" at the bottom of the page.
- Click "Login" in the upper right corner. Then click the "Reset your password" above the Login fields.
- Enter the email address or Username you used to create the account.
- Click "Submit."
- A link will be sent to your email address with instructions on changing your password.
- If you have received the reset link and still cannot access your account, please contact the registry coordinator at: info@ds-connect.org.

- The Password needs to be 12 characters long.
- Make sure you include one upper case, one lower case, one number and one symbol in the Password.
- Make sure there are no spaces between the characters.
- Log in to your account.
- Click the "My Account" menu item in the upper right corner of the screen.
- Click the "Edit" link.
- Enter your "Current Password".
- Enter the new password in the "Password" field.
- Make sure it is 12 characters long and include one upper case, one lower case, one number and one symbol in the password.
- Make sure there are no spaces between the characters.
- Re-enter the new password in the "Confirm Password" field.
- Click the "Save" button at the bottom of the page.
- Log in to your account.
- Click the "My Account" menu item in the upper right corner of the screen.
- Click the "Edit" link.
- Optional - Enter your "Current Password" if you are changing your password or email address.
- Enter changes to any of the other fields on the page.
- Click the "Save" button at the bottom of the page.
- Due to security requirements of the registry, users cannot re-use a Password that has been used in the four previous password settings. This error message appears when you are attempting to change your password and have chosen a password that has been used previously in your account.
- Tip: When updating information in your account do not enter a password into the field for "Current Password" unless you intend to change your password or email.
- If you receive this error message, you need to choose a new Password which meets the password criteria and has not been used in the previous four password settings.
- Note: Any information that has been entered and changed on this page may be removed if the password is incorrectly entered. You will have to re-enter all the information.
- This message appears when you have not logged into your account for over 12 months.
- Log into https://www.ds-connect.org immediately to prevent your account from being deactivated.
- You will also be prompted to change your password.
- Once your account expires, you will not be able to log into your account, and you must contact the DS registry coordinator (Email: info@ds-connect.org) to have your account re-activated.
- Log in to your account.
- Click the "My Account" menu item in the upper right corner of the screen.
- Click the "Edit" link
- Enter changes to the Birthdate field.
- Click the "Save" button at the bottom of the page.
- Log in to your account.
- Click the "My Account" menu item in the upper right corner of the screen.
- Click the "Edit" link
- Enter your "Current password".
- Enter the new email address in the "Email address" field.
- Click the "Save" button at the bottom of the page.
Taking part in DS-Connect is completely voluntary-it is your choice. You do not have to join this Registry. If you decide not to join, you are still a member of the DS community. You can continue to receive health care and other support even if you decide not to join DS-Connect.
